Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Financial Policies

Esthetic and restorative dentistry are excellent investments in an individual's well-being. We firmly believe the financial considerations should not be an obstacle in obtaining the treatment you desire. We are sensitive to the fact that each patient has different needs and have several payment options available so that treatment is accessible to everyone. All financial discussions and arrangements are kept strictly confidential.

Dental Insurance

Most insurance plans are accepted. If you are covered by a dental insurance plan, we will gladly file your insurance forms for you with a request to have the benefits assigned to the office. Co-payments are required at the beginning of treatment.

No Insurance? No Problem!

Yorkshire Dental gladly accepts new patients that do not have health insurance. We realize that many business owners pay dental costs out-of-pocket and we are happy to work with individuals, families, and small business owners that prefer to pay dental costs by cash, check, or credit card. 

Dr. B is kind and very skilled. I needed fillings and she was booked full in the summer, so she scheduled me on her lunch hour and took care of my teeth. I just went back for other fillings and her level of care is quite high. I was also taken to the exam room right away. There is hardly any wait time each time I go.

After not going to a Dentist in over 10 years I was suffering from sever pain from my wisdom teeth. My first appointment was for a cleaning and x rays to see the status of my teeth and to get a plan for removal of the wisdom teeth. After having the Dentist go over the X rays I apparently had 3 cavities as well that I had little knowledge of, so they got me scheduled for 3 fillings, then for all 4 of wisdom teeth extractions. Within less than a month all my dental work was taken care of and now I am no longer in any pain and I can eat without worry. Thank you very much for making this a much better experience than I ever thought it would be.

So compassionate and loving. These ladies do a phenomenal job with adults and children. Two of my children had to have teeth extracted and they went above and beyond for the kids. They made sure they knew everything that was going on and made an environment so pleasant for them. Such a welcoming environment! I am so glad we chose Dr. B and the fabulous team she has!

Great place to get your teeth cleaned and worked on. The hygienist was extremely kind and careful with my more sensitive teeth during the cleaning. Even got X-rays taken and all of it done in less than an hour. They are all very kind and considerate of the people they are taking care of in there. I don't even have dental insurance and the prices were reasonable enough that I was able to pay for it all in full. Highly recommend going here.
